To have the most up-to-date information from IEPWriter in PowerSchool SIS and to correctly and accurately report student data to PDE, the following updates to student IEP/GIEPs need logged into the Student Enrollment Form.
- Primary exceptionality changes
- Placement or building changes (excludes NACA but includes placement to/from institutions)
- Newly identified IEP + GIEP
- IEP Exit from Special Education (mostly speech)
- GIEP Opt-Out
- New 504 accommodation
- Exit from 504
1. Student Number (or LEA ID in IEPWriter) is the information used by the form to retrieve student details from the system.
2. Date of Change should be entered as the effective date of:
- new placement or building
- new IEP/GIEP/504 status
3. Is this Change Temporary? should be Yes if the student's time at an institution is temporary and the student is expected to return to the original or have a more permanent placement afterwards.
5. Type of Request for Student Services will be Enrollment change or internal transfer - NASD accountable for student
6. Change Reason determines the following questions to be answered to complete the form. Please reference the below table for each option:

You will receive a confirmation email once the form has been submitted successfully. If a change needs to be made to one of the field entries, please reach out to the Registration/Data Team members. Once the enrollment update is processed, you will receive an email notification with the update.
